IgG4 lymphadenopathy- Type 2: reactive follicular hyperplasia-like

This is a 27-year old female with history of hypothyroidism and psoriasis who presented with incidental enlargement of a right axillary lymph node measuring 3.7 cm in greatest dimension. The excised lymph node showed hyperplastic follicles with IgG4 positive plasma cells in the germinal centers and interfollicular regions. IgG4/IgG ratio was more than 80%. Increase in IgG4 positive plasma cells with >40% IgG4/IgG ratio is suggestive of IgG4 lymphadenopathy (Type 2: reactive follicular hyperplasia-like).
