This was a 68 year old male with new IgM kappa paraproteinemia.  The bone marrow aspiration showed mixture of small lymphocytes, plasmacytoid lymphocytes, and plasma cells. Bone marrow biopsy showed a small lymphocytic collection which was positive for CD20. Plasma cells were increased in number, arranged singly, and arranged in small clusters. Most of them were kappa light-chain positive with only rare lambda-positive cells. These findings were consistent with lymphoplasmacytic lymphoma.
