An 85 year old woman with a past medical history of endometrial serous adenocarcinoma presented with thrombocytopenia and macrocytic anemia, which prompted clinical work up for a myelodysplastic syndrome. The set of images includes the touch prep and core biopsy of a plasmacytoid metastatic lobular carcinoma of the breast to the bone marrow. Of note, CD138 is not specific for plasma cells and can highlight epithelial neoplasms as shown here.
