A 27-year-old patient from Cote d'Ivoire known to have sickle cell anemia presented with suspicious lesions of cutaneous leishmaniasis. However, a blood smear demonstrated trophozoites of Plasmodium falciparum. These have a vacuolated, thin blue cytoplasm with a dark nucleus.
