Bilobed lymphocytes

A 36 year old, female individual was referred due to lymphocytosis (ALC=5400/μL) without disturbance of red blood cell or platelet markers. She didn’t report signs of infectious disease the preceding days and was not on any medication. Her history was only remarkable for smoking 1,5 packs/day. The peripheral blood smear was remarkable for increased (65% of LYM, 39% of WBC) medium sized lymphocytes with mature, bilobed nucleus. Flow cytometry identified the population as polyclonal B-lymphocytes. This is a case of Persistent polyclonal B-cell lymphocytosis (PPBL).
