86 year old female presenting with acute kidney injury. Serum lambda lightchain 3,710.00 mg/L with no detectable paraprotein on immunofixation. Bone marrow biopsy demonstrating 30% lambda restricted plasma cells and notably multiple Mott cells and extracellular immunoglobulin deposition.
