51 year old male presented with complaints of fever since 2 months with multiple splenic lesions with retroperitoneal lymphnodes. 

Culture and sputum was negative was newly diagnosed as retro positive .

PET CT showed multiple hypermetabolic marrow lesions seen involving the axial skeleton. Multiple hypermetabolic left cervical bilateral left cervical and bilateral subpectoral and mediastinal nodes were also noted. On clinical suspicion of lymphoma bone marrow aspirate was reported elsewhere with maroow plasmacytosis of 15% and bone marrow  biopsy was done and sent.

Biopsy showed partly preserved marrow spaces with 2- 3 intertrabecular spaces infiltrated by dense sheets of plasma cells along with prominent epitheliod cell granulmas admixed with histiocytes and lymphocytes. 

Ziehl Neelson stain was performed  yielded many slender long acid fast bacilli consistent with tuberculous etiology.
