82 year female, having pancytopenia with macrocytic anemia with Hb 8.0 g/dl, WBC 5450/ul and Platelet count of 76000

Peripheral smear shows macrocytic rbcs and hypogranular granulocytes and platelets anisocytosis and anisogranularity.

Bone marrow reveals hypercellular fragments with increased megakaryocytes and predominantly monolobated/hypolobated forms.

Cytogenetics of bone marrow sample showed 46,XX,del(5)(q13q33)[14]/46,XX[6].

Deletion 5q is a recurrent, non-rendom abnormality described in myeloid neoplasm such as MDS or AML. The prognosis in such MDS patients is generally good.
