Prolymphocytic progressive CLL/SLL

47 years old Iraqi male presented with LAP & splenomegaly.

Cbc showed anemia with HB 8 g/dl, WBC count 75000/l, platelet count wthin normal.

PB film & BM sowed infiltration by prolymhocytes forming about 92% of ANC.

Immunophenotyping showed positove expression of CD19, CD20, CD5 , FMC7 & CD79b.

FISH study was negative for cyclin D1/IGH rearrangement while positive for TP53 del in 80% of cells.

IHC stain also negative for Cyclin D1.

So the diagnosis of mantle cell lymphoma variant was rule out and the diagosis of prolymphocytic progressive CLL/SLL was given according to WHO classification (HAEM5).
