37 year-old man presented with pancytopenia. A peripheral blood smear showed marked rouleaux formation with clumping of leukocytes and scattered botryoid neutrophils wit platelet clumping. A Bone marrow aspiration and biopsy showed botryoid neutrophils with variably vacuolated cytoplasm.
