A 70-year-old male with multiple relapsed IgG lambda myeloma developed ptosis of his left eye with double vision. MRI brain showed a left midbrain tegmentum enhancing lesion. CSF cytospin (May-Grünwald-Giemsa stain, x100 objective) showed plasma cells. He passed away with progressive disease.
