71 years old Female patient with history of persistent anaemia with ascites predominant lymphocytic, On ATT with mild renal impairment, The bone marrow is cellular and shows cells of all lineages with dysmegakaryopoesis and mild dyserythropoiesis. Blasts are 3%. The bone marrow biopsy (~ 25% cellularity) showed all hematopoietic cells with adequate megakaryocytes. Cytogenetic analysis of unstimulated bone marrow aspirate culture shows - 46,XX,del(5)(q22q34)[20]. Del(5q) is seen in 10% cases of MDS and isolated del(5q) is associated with a good prognosis except when it is in combination with TP53 mutation.
