A 39-year-old female with multiple mieloma in complete remision after peripheral blood stem cells autologous transplantation and allogenic bone marrow transplantation presenting with dyspnea and pleural effusion. Thoracocentesis was performed and the resulting pleural effusion was very bloody and highly cellullar. In the cytological examination 65% were immature plasma cells named plasmablasts with one nucleolus, finely dispersed chromatin and/or irregular nuclear contour, binucleate cells and mitosis were identified. Myelomatous pleural effusion was confirmed by flow cytometry method. Massive plasmablastic pleural effusion as only manifestation of relapsed multiple mieloma. Cytological examination of pleural effusion. Plasmablastic cells with pleomorphic nucleis and prominent nucleolis. Mitosis and binucleate cells.
