Peripheral smear from a 29 years old woman with SLE history presented to emergency department because of tonic clonic seizure .

Multiple helmet cells, fragmented cells, low platelet and polychromatophic cells in peripheral smear were compatible with microangiopathic hemolytic anemia.

Smear findings and clinical features of this patient were suggestive for TTP.
