Hereditary pyropoikilocytosis is a severe form of hereditary elliptocytosis. The peripheral blood film is from a 4-month-old male child who presented to the clinic with fatigue and general weakness. A complete blood count showed anemia (Hb=8.10g/dL, MCV= 71fL) and slightly increased WBC (10.51x109/L) and Platelet count (513x109/L). The peripheral blood film showed marked anisopoikilocytosis characterized by increased cigar-shaped elliptocytes, tear-drop cells and schistocytes with few hypochromic microcytic cells.
