Bone marrow examination revealed a hypercellular marrow with blasts with blue-grey cytoplasm with microvacuolation (“string of pearls”) around the periphery, with distinct tail-like protrusions (pseudopodia). Mild trilineage dysplasia was noted without expanded myeloblasts, consistent with concurrent CMML-1.
