The peripheral blood smear shows an erythrocyte containing a mature schizont of Plasmodium vivax, composed of multiple intracellular merozoites. Each merozoite has a compact chromatin dot and scant cytoplasm. This segmented asexual stage precedes rupture of the infected erythrocyte and release of merozoites, which subsequently invade new reticulocytes.
