Intracellular and Extracellular Leishman-Donovan Bodies in Bone Marrow Aspirate: Morphologic Diagnos

Bone marrow aspirate smears demonstrate numerous Leishman-Donovan (LD) bodies in both intracellular and extracellular locations. The amastigotes are identified as small round-to-oval organisms measuring approximately 2–4 μm, each exhibiting a characteristic nucleus and rod-shaped kinetoplast, imparting the classic “dot-dash” appearance. The images highlight a spectrum of morphologic findings, including heavily parasitized marrow histiocytes containing abundant intracellular amastigotes and free extracellular forms dispersed within the marrow background. Extracellular LD bodies may be encountered following rupture of infected histiocytes during smear preparation. Recognition of these characteristic intracellular and extracellular forms on routine bone marrow examination permits rapid and definitive morphologic diagnosis of visceral leishmaniasis (kala-azar).
